Health surveillance and epidemiological investigation

The development and maintenance of systems and processes that enable detection, identification, and tracking of health threats, including disease outbreaks and adverse events.

Cross-sector community collaboration

The coordination necessary to engage community-based organizations and social networks through collaboration among state agencies and their partners in order to return to routine delivery of services effectively and efficiently.

Social capital and cohesion

The degree of connection and sense of belonging among residents, including social networks among individuals, neighbors, organizations, and governments.

Incident management

The ability to establish and maintain a unified and coordinated operational structure that appropriately integrates all stakeholders and supports the execution of core capabilities and incident objectives through information sharing, strategy development, and resource management

Information management

The ability to develop systems and procedures for communicating timely, accurate, accessible, and appropriate information and alerts to the public using a whole-community approach.

Prehospital care

Care provided by emergency medical services, including 911 and dispatch, emergency medical response, field as-sessment and care, and transport to a hospital or between healthcare facilities.

Hospital and physician services

Care for patients who are formally admitted to a hospital or other institution for inpatient treatment.

Long-term care

A continuum of medical and social services, including skilled nursing and rehabilitation, designed to support the needs of people living in residential-care settings with chronic health problems that affect their ability to perform everyday activities.

Behavioral healthcare

The provision and facilitation of access to behavioral health services, including medical treatment, substance-abuse treatment, stress management, medication, and social-services networks.

Home care

Environmental monitoring

The systematic collection and measurement of environmental specimens (air, water, land/soil, and plants) to analyze the presence of an indicator, exposure, or response (warning and control). This includes monitoring the environment for disease vectors.
